Rates/Insurance

Mobilize Physical Therapy is an Out of Network Provider, except for Medicare. Payment is due at the time of the appointment.  Many patients have health insurance plans (PPO) that will reimburse a portion of your costs for out of network treatments after their deductible is met. Your physical therapist can provide you with a claim form to submit to your insurance. You can complete the form below to verify your insurance coverage.  Our rates are indicated below. Home visits will include a $50 delivery/convenience fee. This is to cover the therapist's commute time and preparation of equipment to be used for patient's appointment. 

 

Acceptable Methods of payment include Credit Cards, Zelle, Venmo, and FSA (flexible savings account) and HSA (health savings account). Using your HSA or FSA account will save you approximately 25% in taxes.

Direct Access

New Jersey is a direct access state, which means you do not need a doctor’s referral to see a physical therapist! As experts of the musculoskeletal system, getting access to a physical therapist first can save you time going to doctor's and specialist's appointments, imaging, and taking prescription drugs when manual therapy and specific exercises may be your best options.  Your physical therapist will review your medical history and complete a thorough examination to develop a plan of care that is right for you.    If your complaints are beyond the scope of our practice, or you are in need of medical imaging; your therapist will refer you to the appropriate medical doctor.
